The Wellbeing Service
Commissioner: Lincolnshire County Council working in partnership with Lincolnshire Community Health Services NHS Trust
Date commenced: April 2014
Aim: accident and illness prevention, early intervention and reablement, promotion of health, safety and wellbeing, reducing attendance at A & E and hospital admission.
Service description: The Wellbeing Service helps people over the age of 18 to address issues compromising independent living, health and wellbeing, eg: mobility, accessing benefits, social isolation and transport.
This is achieved via:
- assessment at home
- one-to-one caseworker support
- provision of equipment (including TeleCare/lifeline personal safety alarm)
- minor home adaptations
- Stay Safe (monitoring and alarm response)
- Home Safe (supported hospital discharge)

Home Safe discharges
Stay Safe cases have been identified where ambulance call-outs have been avoided, saving the health sector £123,895.**
A total of 3,544 Wellbeing Service and HART customers transported home from hospital where admission via A & E is inappropriate /avoidable, or to facilitate hospital discharge.

Your Wellbeing Service is absolutely fantastic. I have had on and off support for five years (other services) and your service was the only one which made a difference … I have High Functioning Autism and not one service has understood my needs or my way of thinking, however your service has made a huge difference.
Our support worker was very helpful and empathetic. She ordered the grab rails in the morning and they were installed the same afternoon. The pendant pager is the right choice for me, as we just needed a reliable method of communication inside our home, without involving anyone from outside. I now have more of a sense of balance and security.
Because the Wellbeing Service is structured in a totally holistic way, it has had an immensely beneficial impact on my wellbeing. I can now LIVE (not merely exist) in the home that means so very, very much to me …
I think it would be hard to improve the service. I found everyone helpful, friendly and non-judgemental. I feel so much safer with this equipment (key safe and lifeline) installed in my house.
